Please mark your order choices on the ticket and the stub. Detach the stub. You will need to present it when you pick
up your order. Send your order form and your check to Warrick Humane Society, PO Box 82, Newburgh, IN 47629. If you want to
order with a credit card, call Home Away Pet Lodge at 490-7011. Pre-orders may be picked up at the food booth after 3 p.m.
(not noon as it says on the ticket stub.)
